How to get started – Complete and submit the Independent Affiliate Enrollment Packet with DocuSign. Inside the packet you will find the health care plan selections. Click the enrollment application button below.
- The applicant completes the SB/A CoOp Membership Agreement and becomes a member of the SB/A CoOp with no fees required.
- The applicant completes the Independent Affiliate Agreement for Smart Finance affiliation. Your 1099 affiliation is immediately approved.
- Select ONE plan only on the enrollment application.
- Complete the IRS W9 form and ACH banking information.
- After clicking FINISH, an email response from DocuSign will be sent to you.
- In two weeks, the affiliate will receive a welcome package and instructions plus the member medical ID via email.
